Cooperators - How do I claim for Audiologist?

Cooperators claims can be submitted two ways.  

You can go online to www.cooperators.ca and download a claim form to submit by mail.
You can also call the Cooperators to activate your online access to their Benefits Now app.  Through a secure login, you can access general information about your coverage and specific information about your extended health care, dental and health spending account claims.  You'll also find information about dependents, printable Extended Health Care and Dental claims forms, and a glossary of insurance terms.  You can also take advantage of e-claim submission of your health and dental claims through Benefits Now.
If you choose to go online, you will have to make an initial first call to Cooperators at 1-800-667-8164 to obtain a temporary password. 

You can also submit claims with your mobile by downloading the mobile app
